　　2     Business license, registration certificate or approval document on establishment of foreign-funded enterprise (original and copy)

　　3     Valid passport and residence permit (original and copy)

　　4     Employee’s card (original and copy)

　　5     Work Permit for Foreigner (original)

　　Note: In case the attached pages of the Work Permit for Foreigner for renewal, modification or inspection are used out, a piece of two-inch recent photo shall be submitted for making of new work permit; in case other contents of the Work Permit for Foreigner are changed, the relevant supporting materials shall also be submitted.
