Instructions on social security card
If the social security card is lost, damaged or demagnetized, the card holder should report the loss, damage or demagnetization first before taking his/her ID after five working days to collect a new social security card. The new social security card can be collected by someone else, who must bring his/her own ID and the ID of the social security card holder. 3. If a child loses his/her social security card, report the loss first before the person collecting the new social security card for the child taking a 1-inch photo and household registration book of the child and his/her own ID to have a new social security card for the child. Those who haven’t got the social security cards due to the incomplete personal information (redundant information or no photo is available), after the personal information is complete, can deliver a 1-inch photo to window 1A27. After 3 months of the delivery, he or she should take the ID, or someone else should the ID of both, to collect the social security card. 6. Each social security card can be held by only one person. The card holder can use the card in the designated pharmacy shops or hospitals within the city. Those who are not included in the Urban Workers Medical Insurance Program or the Urban Residents Medical Insurance Program are not able to use the cards in the pharmacy shops or hospitals. 7. If the social security card holder wishes to alter the code of his/her card (for the first time), he or she must personally go to the designated pharmacy shops, hospitals or window 1A-29 in the Administration Service Center with his/her social security card and ID. Those who wish to report the loss of card code must go to window 1A-29 in the Administration Service Center with his/her social security card and ID to reset the code. 8. Those included in either of the above-mentioned programs, when seeing a doctor in the clinic or is received as a resident patient, must take the social security card (those who are injured at work not included) 9. The social security card is limited to the use of the card holder only and cannot be lent to others. |
